Note: The Visiting Nurse Association of Central Jersey Community Health Center, Inc. (CHC) is pleased to announce that it will be permanently relocating its adult and pediatric primary care center, currently located at 176 Riverside Avenue, Red Bank, NJ 07701, to a new facility at 188 East Bergen Place, Red Bank, NJ 07701. The move will take place on Monday, December 4, 2017. The CHC Red Bank team of staff and providers, which includes Dr. Prince Ukonu, a family medicine specialist, and Karen Anderson, Family Nurse Practitioner, look forward to welcoming patients to the expanded and more conveniently-located facility. All current patient records will be transferred, and new patients are always welcome.

Who is our staff? Our certified, multi-lingual healthcare professionals include physicians, advance practice nurses, registered nurses, certified nurse midwives, social workers, certified behavioral health counselors, and registered dietitians who will make you feel comfortable the moment you walk in the door.

What about payment?
As a National Health Service Corps Site we promise to serve all patients, offer discount fees to patients who qualify, not deny service based on a persons: race, color, sex, national origin, sex, disability, religion, sexual orientation and inability to pay.

As a federally qualified health center, VNACJ CHC accepts Medicare, Medicaid, commercial insurance, self-pay, and Workers Compensation. We also provide a “sliding fee” scale to assist under or uninsured patients in need. Specialized services, such as preventive health screenings are available to clients who designate the CHC as their primary health care provider.
