Keansburg and Keyport CHC Combine to Continue to Offer Quality Primary Care
As a result of recent staff changes, we are combining the Keansburg CHC site and the Keyport CHC site so that we can continue to offer the full range of services for our CHC patients. The newly combined center is completing renovations and is located in Keyport at 35 Broad Street. Dr. Lewis has moved her practice from Keansburg to the Keyport site, where she will continue to care for her primary care patients. She is no longer seeing patients in Keansburg and that site will not remain open. No action is needed to transfer your records.

Children attending the Keansburg public schools also have the option of getting care for illnesses and help with conditions such as asthma while at school. If you have not already enrolled your child in these Student Health Services or updated your child’s insurance information recently, please see the enclosed enrollment forms. Please return these forms to the school nurse.

Who is our staff? Our certified, multi-lingual healthcare professionals include physicians, advance practice nurses, registered nurses, certified nurse midwives, social workers, certified behavioral health counselors, and registered dietitians who will make you feel comfortable the moment you walk in the door.
What about payment?
As a National Health Service Corps Site we promise to serve all patients, offer discount fees to patients who qualify, not deny service based on a persons: race, color, sex, national origin, sex, disability, religion, sexual orientation and inability to pay.
As a federally qualified health center, VNACJ CHC accepts Medicare, Medicaid, commercial insurance, self-pay, and Workers Compensation. We also provide a “sliding fee” scale to assist under or uninsured patients in need. Specialized services, such as preventive health screenings are available to clients who designate the CHC as their primary health care provider.
