About Us

Visiting Nurse Association of Central Jersey Community Health Center Inc. (VNACJ CHC), a Federally Qualified Health Center, has provided comprehensive primary and preventive health care for infants, children and adults of Monmouth County for more than 20 years. The VNACJ CHC’s are conveniently located  in Asbury Park, Keansburg, Keyport and Red Bank. In 2014, staff provided more than 28,000 visits to more than 9,000 patients.

With the support of the 2020 Vision Campaign led in collaboration with VNA Health Group’s Children and Family Health Institute, our services and resources are designed to provide “one-stop-shopping” for your child’s community health needs, creating a seamless experience between parents and clinicians.

Our History

The CHC opened in 1994 in Asbury Park to improve the health and well-being of individuals by providing high quality, cost effective, comprehensive health care services. In 2003, the VNACJ CHC became designated as a Federally Qualified Health Center (FQHC), independently governed 501 (c)3 guided by a dedicated Board of Trustees of which more than 50% are patients of the centers. In 2001, services were expanded to include HIV primary care and early intervention services. Later, in 2005, prenatal, nutrition counseling and behavioral health were all expanded to the CHC’s scope of services. Lastly, a much needed dental suite was added to the CHC of Asbury Park in 2011.

Our Mission

The mission of the Visiting Nurse of Central Jersey Community Health Center, Inc. is to improve the health status of individuals from Asbury Park, Keyport, Red Bank, Keansburg and surrounding communities. We accomplish this by providing high quality, cost effective, comprehensive health care services by qualified, culturally sensitive and competent staff.
