The Visiting Nurse Association of Central Jersey Community Health Center, Inc. (VNACJ CHC) has been an integral part of Monmouth County for more than 27 years, providing comprehensive primary and preventative care for infants, children, and adults.

The VNACJ CHC’s team of expert physicians and nurse practitioners specializes in primary care for the entire family, as well as prenatal and gynecological care, disease prevention, behavioral health, dental care, chiropractic care, podiatry, lab testing, and more. The VNACJ CHC’s breadth of services encompasses a wide range of high-quality, cost-effective, comprehensive health care services.

As a Federally Qualified Health Center (FQHC), VNACJ CHC provides care at a sliding scale cost—meaning that quality of care is never dictated by ability to pay. Ensuring equitable access and overcoming barriers in the healthcare system is central to VNACJ CHC’s approach to care. 

VNACJ Community Health Centers are conveniently located in Asbury Park, Freehold, Keyport, and Red Bank. VNACJ CHC works across its four locations to fulfill its mission of building healthy communities and enhancing well-being. With a diverse and multilingual team of professionals, VNACJ CHC aims to reflect the New Jersey community, focusing on expanding access to high-quality healthcare in a trusted and inclusive environment. 

This health center receives HHS funding and has Federal Public Health Service (PHS) deemed status with respect to certain health or health-related claims, including medical malpractice claims, for itself and its covered individuals.
