VNACJ CHC team includes fully licensed physicians and certified nurse practitioners that specialize in general health care services for the entire family including disease prevention, immunizations, lab testing and primary care.
PRIMARY CARE FOR THE ENTIRE FAMILY
Our physician and nurse practitioners (NP) provide regular check-ups, gynecological exams, chronic disease management and wellness counseling for health issues such as weight, smoking, and cancer prevention. They also care for children from newborn through the adolescent years. services include well- child checks, sick visits, immunizations, school/camp physicals, and management of asthma, diabetes and obesity.
Our Certified Nurse Midwives (CNMs) provide expert and personal care throughout and following your pregnancy. Services include regularly scheduled prenatal and postpartum visits, supportive assistance from Licensed Clinical Social Workers (LCSW), case management, nutritional counseling and reproductive education. Deliveries take place at Monmouth Medical Center under physician supervised care.
RYAN WHITE PROGRAM
(HIV/ AIDS SERVICES)
Our certified team of HIV specialists provide comprehensive primary care for HIV positive individuals. Services include diagnostic testing, case management, medication management and adherence counseling, psychosocial support and life skills counseling, primary care for those living with HIV and supportive service referrals.
Our specially trained behavioral health Licensed Clinical Social worker LCSWs help ensure patients’ cognitive and emotional well-being, including patients who are having difficulty dealing with the normal stress of life, experiencing depression, or unmanageable family or work challenges.
NUTRITIONAL COUNSELING & DIABETES EDUCATION
Our registered dietician promotes positive behavioral and lifestyle changes while helping you build long-lasting, healthy eating habits to improve your overall quality of life and manage conditions such as diabetes and kidney disease.
Our dentists offer a wide range of dental care* for all ages with sliding fee scale prices. The facilities are state-of-the-art, including digital panoramic X-rays.* No orthodontics or major oral surgery.
PODIATRY (FOOT CARE)
Our podiatrists provide routine foot care, diagnosis and treatment of disorders of the foot, ankle and lower leg, including heel pain, infections, and diabetic foot.
Our chiropractors use a variety of physical techniques and therapies to treat and rehabilitate patients who are suffering from disorders and/or injuries of the back or musculoskeletal system.
ADDITIONAL SUPPORT SERVICES
The patient navigators at the CHC assist our diverse population of patients with complex health needs, as well as, connecting individuals to community resources. Some of these services include scheduling appointments, arranging transportation and ensuring that the patients are following up with necessary providers/community resources that pertain to their health and well-being such as housing, childcare, food assistance, domestic violence services, etc.
The CEED Program is available to Community Health Center patients that are uninsured or underinsured and meet certain financial criteria. Under the CEED Program, women can get cervical and breast cancer test such as Pap smears and mammograms.
The CEED Program also offers colon cancer screening for those over 50 years of age.
HEALTH INSURANCE ASSISTANCE
The CHC employs Certified Application Counselors who are available to assist patients in preparing electronic and paper applications to establish eligibility and enroll in public and private health care coverage.
