FREEHOLD TOWNSHIP—CentraState Healthcare System, in a unique collaborative initiative, has partnered with the Visiting Nurse Association (VNA) Health Group, VNACJ Community Health Center, Inc. (CHC), and Rutgers Robert Wood Johnson Medical School through the Family Medicine Residency Program at CentraState to create a new Federally Qualified Health Center (FQHC) accessible to all members of the community, including underserved populations. The new Freehold Family Health Center is opening on March 20 at 597 Park Avenue in Freehold.

The Freehold Family Health Center operated by the CHC will provide both comprehensive primary and specialty care, including geriatrics, offered through the Rutgers Family Medicine Residency Program at CentraState. In the same building, the VNA Children and Family Health Institute will offer community health programs such as WIC, programming by the County Councils for Young Children, and cancer education and early detection programs to benefit area residents. Bilingual staff are available to assist with all services.

Designed around community needs and input, the Center will provide comprehensive primary care to a diverse population, many of whom face significant disparities in health outcomes. Every facet of the Center is designed to serve as a resource for individuals and families, many of whom may struggle to access care, including a location that is easily accessible by walking or bus routes.

Rutgers’ CentraState family medicine resident physician will care for patients at the FQHC for their full residency, an approach that reinforces the importance of community collaboration and prevention initiatives to address the burden of chronic disease and improve the wellbeing of the community as a whole.

In this state-of-the-art environment, patients of all ages will have access to primary and family care along with the many support and health care navigation services needed to keep their families healthy.
