The CentraState Family Medicine Center is Moving to Freehold Borough in 2018!
The Freehold Family Health Center is a unique partnership between the Visiting Nurse Association Central Jersey Community Health Center and the Rutgers Family Medicine Residency at CentraState. The Rutgers physicians care for you both at the health center and when you need admission to CentraState hospital, so that you have the same team of physicians caring for you with no gaps in care.
Services Available for the Entire Family
PRIMARY CARE FOR THE ENTIRE FAMILY
Our physicians and nurse practitioners (NP) provide regular check-ups, gynecological exams, chronic disease management and wellness counseling for health issues such as weight, smoking, and cancer prevention. They also can take care of skin problems including checking for and treating early skin cancers. Our providers care for children from newborn through the adolescent years. Services for kids include well-child checks, sick visits, immunizations, school/camp physicals, and management of asthma, diabetes and obesity.
Our OB/gyn and family physicians work together to provide expert and personal care throughout and following your pregnancy, including being with you for your delivery at CentraState. At the health center services include regularly scheduled prenatal and postpartum visits, supportive assistance from Licensed Clinical Social Workers (LCSW), case management, nutritional counseling and reproductive education.
Our OB/gyn and family physicians and nurse practitioners (NP) provide services including gynecological exams, contraception including IUDs and Nexplanon, initial infertility work-up, and follow-up to abnormal pap smears and bleeding.
CHRONIC DISEASE EDUCATION
Our physicians provide care of chronic diseases including high blood pressure, diabetes, obesity, cancer, mental health, and communicable diseases for people of all ages. Our professionals provide preventative education at all locations.
PODIATRY (FOOT CARE)
Our podiatrists provide routine foot care, diagnosis and treatment of disorders of the foot, ankle and lower leg, including heel pain, infections, and diabetic foot problems.
Our specially trained behavioral health Licensed Clinical Social Workers (LCSW) work directly with your primary care provider to address your cognitive and emotional well-being, including if you are having difficulty dealing with the normal stresses of life, experiencing depression, or struggling with managing condition such as diabetes or overweight.
HEALTHY EATING COUNSELING
Our registered dietitian promotes positive behavioral and lifestyle changes while helping you build long-lasting, healthy eating habits to improve your overall quality of life and manage conditions such as diabetes and kidney disease.
Our physicians and Sports Medicine specialty faculty provide services ranging from pre- participation sports physicals, caring for your sports injuries, and injecting painful knees, shoulders and other joints.
GERIATRIC SPECIALTY CARE
Our geriatric specialty physicians provide care to seniors to ensure they can maintain the most active and independent lifestyle possible. Members of this team also provide care in many area senior living and nursing home facilities.
Our physicians and Gastroenterology specialty faculty provide extensive care regarding problems of swallowing, the stomach and intestines, and liver.

HEALTH INSURANCE ASSISTANCE
The Freehold Family Health Center employs Certified Application Counselors who are available to assist patients in preparing electronic and paper applications to establish eligibility and enroll in public and private health care coverage.
340B PROGRAM (DISCOUNT DRUG PRICING)
This program provides prescription drugs to eligible patients at significantly reduced prices. Most pharmacies participate.
CANCER EDUCATION & EARLY DETECTION (CEED)
The CEED Program is available to Community Health Center patients that are uninsured or underinsured and meet certain financial criteria. Under the CEED Program, women can get cervical and breast cancer tests such as Pap smears and mammograms. The CEED Program also offers colon cancer screening for those over 50 years of age.
The patient navigators at the Freehold Family Health Center assist our diverse population of patients with complex health needs, as well as, connecting individuals to community resources. Some of these services include scheduling appointments, arranging transportation and ensuring that the patients are following up with necessary providers/community resources that pertain to their health and well-being such as housing, childcare, food assistance, domestic violence services, etc.
