Healthy Families
Healthy Families provides a link to community for pregnant women and assists them with obtaining necessary supplies. Trained Family Support Workers visit expectant women in their homes during pregnancy, or up to three months following the birth of the infant. they provide education on parenting through the use of a structure curriculum and assessment tools. Nurse-Family Partnership
Nurse-Family Partnership (NFP) pairs a nurse with a first-time mother in Monmouth and Middlesex County to provide assistance and guidance throughout the maternity process and early childhood years. Trained NFP nursing professionals with an extensive background in maternal child health provide weekly or biweekly visits with the goal of improving pregnancy outcomes; improving child health and development, and improving the economic self-sufficiency of the mom and family. Services can include parent education, health care, child protection, education, criminal justice, mental health, welfare and public assistance, and taxes. Pediatric Home Care
Our comprehensive pediatric home care services range from hourly assistance with daily living activities to 24 hour care; from short term, post operative nursing care and rehabilitation to specialized home infusion therapy. Contact us for additional information.

School Based Youth Services
VNA Health Group school based programs offer primary health care to more than one thousand students each year. Our school based nurse practitioners provide care at five school districts throughout Monmouth and Ocean counties. Services include immunization and record review, scoliosis screening, hearing and vision testing, health education, well visits, prescriptions, school physicals, TB testing and day care center services. Special Child Health and Early Intervention
Starting with a complete child/family needs assessment and evaluation, our case managers create a plan to address the child’s unique medical, dental, education, developmental and rehabilitation needs. Services include early intervention services, school based concerns, nursing services, social services, community resources, prescription drugs, medical insurance, medical bills, therapy services, mental health resources, medical equipment, and nutritional counseling.
